Instructions for returning defective product for repair or exchange

Complete this Return Authorization Form by providing your personal and product information. Once completed, print a copy of the email confirmation you receive after submitting the form and include it with the returned product. Keep a copy for your own records.
Please write the Return Authorization number, (provided in email confirmation) on the outside of the BOX.
Include a copy of purchase invoice in the box, this allows us to determine warranty from repairs.
